Action Replay Sega Saturn Incompatability
I wonder which games don't work with that 4MB Action Replay Cart. I watched a Classic Game Room video and he pointed one out, but I can't read the title of that game. I've also heard that one may also encounter some trouble with the Neo Geo ports, and I've seen it happen on Real Bout Fatal Fury, as the characters glitch out in various ways.

The regular release of Soukyugurentai for Saturn has some glitches in the text bits when played on a US machine with the cart as there were some English bits, but some is incomplete. The rerelease Soukyugurentai Otokuyo does not as all of the text just appears in Japanese. It doesn't affect actual gameplay though, so unless you're reading the text, it doesn't matter. Japanese copies of Vampire Savior can have issues with some older version of the cart. Though there's a fix you can do on the cart to make it work.

More info on that? I haven't seen any glitches at all on my copy of Vampire Savior + 4MB AR.

Action Replay won't save or load while running.
So you'll still need a backup ram cart to fully play games like Dezaemon 2 and Dragon Force.
The console internal region (such as video display mode) also does not change.
NTSC Saturn defaults PAL games to NTSC60.
PAL Saturn defaults NTSC games to PAL50

One of the King of Fighters ('95 IIRC?) won't work BC the cart was also a ROM (not RAM) cart & had pieces of game data/code on the cart as well. So obviously the AR won't run it.

It only happens if you have an older revision of the cart, or one that hasn't been 'fixed'. Ones without the port are all later enough to be fine I think.
