haha, i actually AM stuck on an island. with 200 people. and no store. access by ferry only...
and sadly i didn't bring my genesis

but, if i had brought it and could only take one game, i'd pick shadowrun. playing forever just to earn money and karma, especially by running the matrix, is strangely soothing to me. the music, writing style, and gritty graphics put it in my top 3 of all time.

Mother fucking Contra Hard Corps. There are few games out there that are able to give the instantaneous gratification and thrill that Hard Corps pumps into your veins. The shmup selection is pretty wild too, it would be painful for me to turn my back on Thunder Force and MUSHA.
Although there are several great RPGs to pick from, I would rather have something with high pick up and play value. Being stranded on an island and sharing your final moments with Phantasy Star IV doesn't sound like the worst thing that could happen. But for me RPGs can get boring after several plays unless it is like your favorite game ever, but the best arcade-esque games seem to retain infinite re-playability.
Hearing Fang say "LET'S ROCK" as the life leaves my body and that crushing guitar riff of the first stage sends me off like viking sounds like the way to go.

Well, on a purely technical level, import Genesis games aren't Genesis games. I mean, the topic does say "Sega Genesis." It doesn't say "Sega Genesis/Mega Drive."

Agree with Phantasy Star IV.
Haven't played Hyper Stone Heist but imagine it'd be high on my list considering I loved TMNT Arcade & Turtles in Time.
I might be tempted to bring Eternal Champions... I still suck after decades, perhaps I'd finally be good if it was the one game I owned

Hyperstone heist is basically a remix of Turtles in time. If you love one, you love the other.

I think I might have to go with Shinobi 3. It's probably one of the most nostalgic inducing games of my life. It was Kindergarten, lived in Tennessee for about a year or two on a military base when my dad was in the navy, and I just have so many awesome memories of those times and some great friends I still wonder about today. This is when I was introduced to Doom, and the SNES made its mark with Donkey Kong Country, my dad and his friend played some games with me which was a rarity from ActRaiser, to Earthworm Jim, Lemmings, Super Ghouls n' Ghosts, etc.
But yeah, all my friends had a Genesis back then and Shinobi 3 was the game I always played at my friends. I'd wake up before them and start playing it in their living room haha. They could never get too far, so I was the go to man for the job. Played it constantly.
Good times. Streets of Rage 2-3 were big ones for me too.
