Back on topic, OP mentioned a cave, which makes me think of Gravitron or Solar Jetman. These are not shmups, but they are games with spaceships in caves, and you do shoot.

I don't mine the snipehunt in this case, it's fun to look through obscure shmups and be reminded of a few you've forgotten. I just fired up mame to play some Side Arms Hyper Dyne. 
Edit: What about Thunder Cross? It's a cousin to Gradius but with lettered item drops.

Damn, talk about a needle in a haystack. There's a lot of shmups out there.
Did it have Vertical stages too? If so, it would explain why you are having a hard time finding it.
Try this one though. It matches your description fairly well (besides the vert stages).
