Just think of pretty much any game in history that is notorious for its difficulty and you'll find all the cheap deaths you could ever ask for. More often than not calling a game really hard is really just a nice way of saying, "it's full of cheap bullshit"
There's a reason I don't really gravitate towards hard games much these days. I'm sort of tired of getting screwed by dick game designers.

I've never though that was cheap, mainly because if you are knocked back and die, it's because of a very predictable enemy pattern, you are not killed by skeletons throwing bones but by medusa heads or any other hazard. I think that if you are being knocked back, it's because you did something pretty wrong, so the game kills you. I hate the "leap of faith" in any game, even great games. The Super Star Wars series has a few of those.
In the amazon level of Duck Tales, right before Launchpad McQuack lends you a line, there is a bee that I swear can not be dodged. Sucker gets me every time.
There are a few places in Blaster Master where SOPHIA can get stuck, and it is game over. There are also times that if you leave the tank, it's impossible to get back to it.
And although it's not "death" if you are up 21 points in the 4th quarter and playing the cpu in Blitz (from the N64 to the oxbox), you'll lose.

I don't think Castlevania is cheap either. The game is built around punishing you for trying to just blaze through it. You gotta be ready for whatever happens, take your time, think ahead, etc.

Super star wars 2 and 3. God I wanted those to be good, but they're not. The first is average.

Does S.T.A.L.K.E.R can be considered a cheap game or a really hard one? I died like 7 times in a staircase because my enemies can shoot me in the head with NO problem.

I don't think those RE4 QTE's are that cheap, from what I remember you can usually predict when they're going to show up, like when those knight armors attack you, it's pretty obvious that something like that is going to happen.
