Jurassic Park was pretty bad.
There was simply "Jurassic Park" for the nes then again for snes, two totally different versions. Oh wait there was one for the genesis too, again totally different but even more so.
Then there's the Lost World which could be referring to either the awesome arcade game or the not so awesome Saturn game. And for the Playstation?
And Jurassic Park III (which there were about 4 of).
There are others I have failed to mention.

This is a pretty amusing thread.
I was always annoyed by the title Wario Land: Super Mario Land 3 for the Game Boy. In my opinion it should have just been Wario Land, end of story, or had the title/subtitle flipped to be Super Mario Bros. 3: Wario Land. It makes more sense to me that way, and considering that its predecessor was Super Mario Land. 2: 6 Golden Coins, it would have retained a little bit of consistency.
I also have a habit of referring to any Bomberman game as Wario Blast because of the GB title.
How about the Monkey Island series? Certainly not too confusing in comparison to the other examples you've already gone over, but guilty all the same.

I have a Japanese game for my Game Boy that I think is called Adventure Island but I am still not 100% sure about it. What makes it even more confusing is that the more well-known Adventure Island (the NES game) is based directly off of the original Wonder Boy. It shares the same gameplay and levels, but the sprites were changed.
So there's two completely different games both called Adventure Island which are both related to Wonder Boy, one of which literally is a Wonder Boy game (PCE) and one which is a sprite changer (NES).
