Hmm, how about those fast enemies in Dead Space. Anyone who's played the first probably knows what I'm talking about, hehe.
I think MMX5 had one of the biggest "OH #@$%!" moments I've ever seen with a lot of those weird hints to some stuff at the very end... that they ended up doing absolutely nothing with in the later games, man did that piss me off. I guess the same can be said about X2 or X3's subtle hints at the end about the fact that, for the war to stop, Zero needed to die. If Capcom had the balls that would have been one heck of an epic conclusion for the series at some point, X vs Zero. And by that, I don't mean the lame way X6 did things.

Way back when, the very first time I got to the Doomsday Zone in Sonic 3 and Knuckles.
And of course that was after fighting the huge bastard robot at the end of Death Egg, with the freakin' planet in the background.
Blew my young mind the fuck away.
Also, the climax of Mass Effect, with the spacewalk outside of the citadel.

I hadn't read anything online prior to playing the game so I was surprised/kind of disappointed because she is basically your only healer.

I was playing this in a dark room at 1-2 AM and was extremely tired. When Psycho Mantis asks to put the controller down I placed it on the coffee table in front of me.
