I was going to post this (except for the part about not being able to vouch for the games). As far as I know, Golgo 13 is the only character in an American NES game to have multiple sexual partners over the course of each of his games. That's enough right there, imo.

I'd say the second is notably more badass than the first, though. Both include images of a skeleton with a crown of thorns on his head, but as I recall the second one features its head being blown off.

Heavy Barrel (top-down, blast everything shooter).
The Ikari Warriors games (unless you think they're too cute)
Guerrilla War (Che Guevara and Fidel Castro overthrow Batista by means of shooting and blowing crap up. This one may be a little too cartoony, though)
I don't think of The Legend of Zelda as being a particularly manly game.
Also, I see TMNT3, but not TMNT2: The Arcade. How come?
Also, consider Astyanax. The opening cutscene isn't exactly the manliest in the world, but after that you walk around smashing the crap out of stuff with an axe. Also, I recall the music being pretty good.
What about Kung Fu?
Also, it wasn't a very good game, but I think that Dirty Harry should probably be on the list by virtue of the fact that it exists and stars Harry Callahan.
Oh, also, TMNT: Tournament Fighter
Also, if you're going to include the Famicom Splatter House game (which was really pretty cutesy), you might want to include the Famicom Altered Beast. It was pretty terrible, but it was made, and Altered Beast is pretty manly.
Oh yeah, also check out Rescue: The Embassy Mission. One of the few NES games with blood. First, you have to dodge search lights to position your snipers, then you use them to shoot terrorists through some windows, then you rappel down the side of the building and crash through a window, then you go room to room shooting guys.
Mighty Final Fight might count. It's pretty cartoony, but I'd say it's more brutal than River City Ransom.
Oh yeah, Werewolf: The Last Warrior. It's not a great game, but it's not terrible, either. You can turn into a werewolf and climb up walls and stuff, and Karnov is the first stage boss.
