It requires a valid US driver's license and passport, and you must be at least 18 years old, though over 21 is preferred. Austria, Germany, and Switzerland require you possess an International Driving Permit unless your license is in English.
If you want to rent the vehicle in USD, you need to set up the rental service at least two weeks in advance. If you want to leave the country, you need to examine the rental contract to ensure whether you can. Most western European car rental services don't want you to drive east of Germany except to Austria, and some don't want you to drive in Italy. You should also ensure the rental agency or credit card provides auto insurance, because US auto insurance won't apply. All drivers in Germany must legally carry a first aid kid and warning triangle.

My dad travels every week for work. His company rents his hotel, books his plane, and rents a car, and then lets him accumulate all of the miles and benefits. We've paid for multiple international trips through frequent flyer miles and Marriott rewards, and we get some pretty nice cars when we go international. In Germany we got to rent a brand new BMW, with the only miles on it what it took to drive from the lot to the rental agency. I could be alone on this, but having a career that makes you travel a lot and rent cars super rules. My company sent me to a convention and the rental car I requested was unavailable. Instead I got a Honda Odyssey with a V8 engine that purred like a kitten and drove like what I think a Porsche would handle.
Do not "drive it like you stole it", "drive it like it's a rental".

Unnecessarily harsh and most likely fabricated insult. The International Motor Insurance Card is provided by the rental company if you are a US citizen renting abroad.
