As some may know my Fiancée is a (hardcore) Silent Hill fan, she's beat most of the games and is very forgiving when it comes critiquing the later game in the series. We were decompressing after a long week by playing SH4, as we were making our way through the hospital we encountered a large intimidating enemy (called “the Patient”)
As she's wacking him, the most dumbest sound comes out of one of the most badass enemies in the SH series....a BELCH!! (sound clip coming soon)

It has a horrible background noise trying to replicate the Arcade. Getting a Power Pill mutes it until the ghosts change back to normal.

Either Avan's constantly repeated obnoxious laugh from the English version of Valkyria Chronicles II, or Thousand Arm's protagonist's footsteps sound effect when he runs. It sounds like someone's shaking a crappy maraca the whole time you play the game.

Any sound effect that indicates you are low on health/life, similar to the less annoying sound on the The Legend of Zelda. 1941: Counter Attack (arcade) has a very loud sound that seems to overpower anything else going on.

That's what mortally wounded foes say as they lay dying in Golden Axe II. Seriously, you hack a guy to death with an axe or sword and all he can do is let out a disinterested "bleh." No scream; not even a death rattle. Imagine your children gathering around you as you lie on your deathbed and they lean in to hear you speak your last words. You say, "Bleh."
It's such an inappropriate sound for that moment that I can't stand it and it really takes me out of the game.
Another horrible sound effect is the "Ni Ni Ni Ni!" that the guy from Dark Castle (Genesis) says when he gets hurt. It's actually a hilarious noise, but it happens so often in that game that it gets irritating.

Thats a tough one...I suppose any game where you shatter Ice and it sounds like glass. Ive never heard real ice shatter like that.

Well if you want to go that far, crashing cars don't sound anything like TVs/Movies/Games.
