I usually watch a horror movie a day every October, but decided to start a little early this year. I started to think, which ones are the most disturbing / disgusting / hardest to watch? So far, these are the ones I have found:
Possible spoilers
Hatchet: Just about every death scene took the blood and gore to an extreme. I have yet to see Hatchet 2 to see how much farther they could take it.
Hostile: The scene where the Asian woman had a blow torch taken to her face, and had her eye cut out of her half melted face. I found this scene incredibly difficult to watch. And what happens after the dude cut her eye out... *shudders*.
Saw 3D: The scene where Bobby falls from the chains he's hanging from, and has no choice but to sit there and watch as his wife boils in the Brazen Bull / oven.

Watched Fulci's "City of the Living Dead" for the first time a while back, and the entrail extraction scene was the sickest thing i'd seen in a long time.
"Cannibal Ferox", which I rented at Blockbuster(!) under the name "Make Them Die Slowly" in the mid 90's has some gruesome images. First time I had seen a penis in a movie that was not a porno, and it gets chopped off and eaten.

A Serbian Film - Everything, just to give you an idea the cut version (4 minutes of film were cut) is rated NC-17 for Extreme Aberrant Sexual and Violent content including Language.
Cannibal Holocaust - The ending and the turtle scene.

Hobo With A Shotgun - The schoolbus scene

Re-Animator - Probably the most perverse use of a living decapitated head, ever.
It doesn't really belong here, but Marilyn Manson's video for "Sweet Dreams" still manages to creep me out. Anybody else feel the same way? It inexplicably manages to just feel like something you should not be watching.

Nothing was quite as disturbing to me as when I got my hands on a VHS of the Nine Inch Nails video series for Broken back in the mid 90s. I got rid of it right after I watched it because I was so disgusted at the time, which might have been a mistake because it's probably worth a fair amount now since it was banned pretty much everywhere. It occassionally exists on youtube still, though it gets taken down all the time due to the extreme graphic content:
