That is a hard decision, their is so many good games. Sonic and Knuckles first, a close second would have to be Streets of Rage 2.

-great music
-good replay value (2 separate characters)
-fun levels

If we do go by playtime, then Sonic & Knuckles would have to be my choice. I played through that game tons of times and I usually played it without Sonic 3 as I firmly believe it is a great game on its own. And for you other Sonic & Knuckles fans, check out these out if you haven't already:

I also played Streets of Rage 2 a lot, but that elevator stage pisses me off to this very day so the game won't get a favorite nod from me as a result.

My runner-up for fave would be Earthworm Jim, which I do consider a genesis game even though it went multi-platform, much in the same way I view Resident Evil 4 and Zelda:Twilight Princess as Gamecube games, the system they were actually designed for.
