It's been a lot of years, but the one that got the most multiplayer time is actually Forgotten Worlds. SFIICE, Cyborg Justice, Sonic 2 splitscreen and Zombies ate my Neighbors got a lot of play time and after we got General Chaos that did in all the others, but still we spent so much time playing Forgotten Worlds that it wins hands-down. Sadly we didn't even know that Gunstar Heroes existed.

Didn't play much sega with friends but when we did it was either NHL 93
or either of the mutant league games

Dont really play Mega Drive with mates too much, every now and again I will play Streets Of Rage (Dont have 2 yeat) or Sonic 2 split screen. Other than that not much.

Usually either Sonic 2 or 3 split screen, Puyo-Puyo (Japanese) or Chibi Maruko-chan: Waku Waku Shopping (Japanese). Back in the day, we also played a lot of Mario Lemieux Hockey, NHL Hockey, NHLPA '93, NHL '94, Road Rash 2, Mortal Kombat, Street Fighter 2: SCE, Joe Montana's Sports Talk Football '93 (Bills vs. Lions, every single game) and Evander Holyfield's Real Deal Boxing.
Yeah, we loved hockey. Still do. NHL Hockey ('92) was the reason myself and pretty much anyone I knew bought a Genesis. We used to have NHL Hockey and Evander Holyfield tournaments all the time in '91-'92/'92-'93/'93-'94.

Streets of Rage 2, Toe Jam and Earl: Panic on Funkotron, Gunstar Heroes, and occasionally Sonic 3 and Knuckles with someone helping me out as Tails.

Streets of rage and Golden axe games. I grew up on brawlers and they still do it for me.

Toe Jam & Earl of course. It's got the perfect combination of cooperation (work together to figure out all the presents without hitting randomizers) and competition (who can find the pieces first/clear the most map squares and level up).
