Sega Genesis and Phantasy Star II. My console doesn't run as well as it once did, but I still love it, and PSII was one of my favorite games growing up. A lot of plot elements that really resonated with me as a teenager. Also, there weren't all that many sci-fi RPGs back then, so I really liked the series for going that route.

The console is the Sega Genesis hands down. I don't have to spend more than a few moments thinking about it. Along with the NES and the 2600 it's what I grew up with and it's the moment I went from liking video games to loving video games. Some of my most vibrant childhood gaming memories come from this machine.
The game is much more difficult. I could name any number of them. If I had to choose just one I guess I'd have to go with my old standby of Wing Commander 2: Vengeance of the Kilrathi on the PC. It showed me games could be more than just a series of obstacles. They could have a serious narrative and be cinematic in scope. It was the first time I considered a game more than just a game and as a piece of actual literature even if by today's standard the plot might be a bit hammy (I do think it's still decently written though).

Console: Sega Model 1 with Model 2 CD
The Sega was my 1st console, my brother got the nes and snes. I have to add the cd as well because they were expensive and my grandma bought me one for my birthday.
Game: Sonic 2; I played this game so much. I would pack my sega up and take it to my grandmas everytime I went.

I would have to say my PS2. I went out and found a 14" Wega to keep it hooked up to (god it looks horrible on my LCD and plasma) and I still plan on buying a bunch more games for it. I may also get another new slim one to have waiting in the wings for when this one will inevitably stop working.
Maybe its just nostalgia at this point, but I think I have had way more fun with this console than any other and even though I have a 360 and PS3, I still find myself playing the shit out of it.

Sega Master System and Alex Kidd in Miracle World. No contest. Sadly I no longer have my original SMS2 and I've never even finished the game on the actual hardware. I still have a lot of time for the Master System. That damn music... so good... so many memories.

Despite the Atari 2600 being my first system, the NES, Sega Genesis (Model 1) and the Super Famicom were where I established my fundamentals. With that being said, the Super Famicom is my most cherished console mostly because that's when/where I discovered gaming culture and Japanese imports.
As for games, I've always been partial to the Breath of Fire, Shining Force and Fatal Fury franchises.

Console: SNES. It's the first console that I owned as a kid, and some of my best memories as a kid were playing that thing. I love it to death.
Game: Castlevania: Symphony of the Night. I always wanted it as a kid, but never got it until I was 15 or so and found it in a BJ's for 16.99. I've beaten that game more times than any other game I own... by a lot.

"Shmup" is far more elegant!
