In terms of what I like, even if its for no real good reason:

4. Sega Saturn official memory card (so helpful!)

That's as far as hardware. There's nothing really software wise that I have a Top 5 for, I could list valuables, but that wouldn't be fun. I could list what I like, but that list changes like every 5 minutes.

out of curiosity whats so helpful about the official memory card i have an action replay 5 in 1 and i think thats the best thing since sliced bread. being able to play import so easily is awesome

I've never owned an AR so I don't know if it works at all the same, but I know when I had a GameShark I manually had to transfer over most games to it (I think Croc was the only one that ever saved directly too it). Most games will save to the official cart directly without me having to go in and manage it. Plus it's actually fairly rare. I only ever see a few on eBay at a time, and it doesn't seem to be included in very many Saturn lots either.

ya you have to manually move the save its not that much of a pain i play mostly import shooters on mine so the region bypass is invaluable, how do you do it a region switch mod? i just save to the internal memory for the game im currently playing and back everything else up on the cart

I don't actually have any imports. When I do start importing I'll likely just buy an ST Key for occasional use. I play quite a few different genres on the Saturn and NiGHTS alone takes up most of the internal space, and I also really like being able to just pop in a game and load a save without messing with file management.
