Damned aligator in the sewer tunnel....Resident Evil 2.
Speaking of, is Claire's version of the game much longer than Leon's, or am I nuts? I hadn't played either for years, so I decided to fire up a PSP and have a go the other day. I ended up saving only twice and beating the game in what seemed like just a couple hours.
Trying to duck under a laser-tripped mine on a bridge linking two modules of a oil rig/evil science lab. Suddenly the wind rocks me over the edge, and I fall into the icy waters below.
Before that, I failed to notice one of the beams right in front of me. The mine exploded in my face.
Cold Fear.

The massive eye head guys? Those guys are nasty. Even with the best Frenzy protection you have to kill them fast and then wait a while for your Frenzy to drop all the way down. You're screwed if you aggro two.
