I would probably say Kung Fu/Spartan X.
It's not really that hard a game, and I've since beaten it, but the game frustrated me to no end when I was younger. I always felt like the game was kind of cheap, but I think it's just that the controls were pretty clunky.
I believe the OP mentioned R-Type. I have the SMS version and never beat it, but it never enraged me the way Kung Fu did.
Also, Toxic Crusaders frustrated me quite a bit. The game is just terrible, but I didn't have many games when we got it, so I had to try to make do.
Other honorable mentions from later generations (That I beat but caused me lots of frustration):
Super Mario 64: A great game, and it's not that hard, but for some parts the camera just makes things a nightmare.
Conker's Bad Fur Day: Also a great game, but I hate the part where you have to go through the corridor with the laser trip-wires connected to explosives. The game's presented on a 2-D screen. There's often not enough context to tell exactly where that uniformly-colored red bar it and if you're about to touch it. Grrrr... That was quite annoying.

I actually beat Ninja Gaiden 1 on the actual NES hardware. I have a retrozone flash cart and was destined to beat it. After about 1 week of studying it, and punching walls, I memorized where absolutely every ninja, witch, cheetah, bird, silver bird (the birds made me want to go kill people....), etc. I sat done one Saturday and beat it (after like 2-3 hours). I actually took pictures of me beating it on the way end screen... In all weird poses. I played the last level over 50 times (yes.. with continuing). And i didn't beat the last boss in one go, I beat the 1st part then died... I also yelled at my girlfriend, punched my leg with the controller leaving a huge bruise, told my best friend he was jinxing me becuz he was playing Croc.... Ill post the pictures later... there great haha!

Ikaruga, Castlevania Dracula-X and Ninja Gaiden 3. I just beat NG3 not long ago and lost about 3 yrs of my life from stress and frustration. Ikaruga, I love the game but I suck at it. I look at those You Tube videos and know I could never achieve that level of skill. And then theres Dracula-X, you have to be God or at least Jesus to beat it. Some of those levels are fucking hard as fuck.

I find the Advance Wars games infinitely frustrating for me. I've played through part of Advance Wars on the GBA and I can appreciate how it is well-made on several levels, but I am no good at it. I am a fan of turn-based RPGs like Disgaea and Shining Force 2, but when I have to win a battle with skill and tactics instead of overpowered characters, I just fall flat. I wisely decided to quit the game before I was left with a pile of broken GBA in my hands.

That's like me and Geometry Wars. Damned videos and strategies make it seem like the game is a cake walk...but I get my ass handed to me all the time. I think I unlocked like 3 achievements for the game and completely stalled out on getting any other ones despite working my ass off trying to get them.
