(Disclaimer: I am not a doctor. Go see a doctor. Preferably a dermatologist if you can manage it. A general physician could misdiagnose it.)
I am leaning towards poison ivy or Schamberg's purpura. About a year ago, I had a similar rash around my ankles that I thought was poison ivy. After a few months and many trips to my general physician, I wasn't getting any better. I went to a dermatologist who took a brief look and immediately knew what it was. The treatment was compression socks and a low dose aspirin to increase blood flow. It cleared up pretty quickly.
If it turns out to be poison ivy, I have had a lot of luck treating it with a hair dryer. For me, it has worked better than any cream out there. The way it works is you apply hot air to the affected area for about 3 minutes. As hot as you can stand it. The itching sensation will briefly intensify then fade away completely. The rash will look very red and angry for a while but the itching will be gone for a good 8+ hours. Repeat daily until the rash is mostly gone. (Don't attempt this unless you are sure it is poison ivy. If it something else like Schamberg's purpura, you could make it worse)

I did something similar with really hot water in the shower. I kept doing it until the itching sensation went away. I'm going to try the hair dryer tonight.

Some of those creams also help dry out the poison ivy. You might have to ask someone who works at a drugstore for help. I remember my mom putting stuff like that on me when I was younger. I used to get it quite often.

Get someone to rub some poison ivy leaves up between your shoulder blades where you can barely reach. I guarantee it will eventually preoccupy you so much that you won't even remember you have anything on your ankle.
