It is terrible. PS1 version has the same cover. That's actually one of the reasons why I didn't buy a CIB copy for the Saturn for 8 bucks because I knew I'd have to look at that creepy ass cover every single time (and I typically do not buy games on CD unless they're CIB). Thankfully the N64 version does NOT have this cover, mostly because it came out later than the other two.

I remember that Foresaken cover art and I always got confused by it, like what the hell was it supposed to be?
