I just don't get how you find someone thinking money shouldn't determine priority as being an idealist, just because you can't think of someting practical that could be used to determine and more fair. I find just being pragmatic or idealistic too limiting. You need a balance of both. The problem is that money WILL determine priority no matter the system you use. The difference would be that right now money = priority is built in whereas in some other system it would be a side way in to changing priority. So saying "money shouldn't determine priority" is akin to saying "the sun shouldn't cause skin cancer". I do pay for insurance, but sometimes I wish I didn't. Insurance companies treat patients like garbage, make me wait weeks and sometimes months to get an appointment and have shitty coverage.
I sometimes flirt with the idea of going back to paying for what medical services I need. A few years back I used to do that and it worked fine. And I got better care because I paid the physicians right away instead of giving my money to a middleman who would then pay peanuts to doctors.
But, well, I always have that what if thinking that I might need a super expensive incredible treatment... so I put up with this shitty service.
Well, I know my opinion doesn't count for much around here since I live in a poor country and my circumstances are unlike most of yours, but it's just my 2 cents.
