If it helps, just take your time on the boss. You don't have to actually kill him you just need to survive. It's actually not terribly difficult if you just don't hit the boss.

I never knew that. I just assumed you had to kill him, and I'm always like "What the hell! I can't catch up! I'm never gonna be able to hit him enough times!"

Are you telling me...that....all of my life....I just had to survive? I thought I had to kill him!? Why would they do that to me!? I have never been able to beat Sonic the Hedgehog on that principle ALONE!!!

I always found the boss confusing too. I figured that I had to catch up and thought I was terrible at the game.

Ugh. So this thread gave me a little motivation, and I decided to turn on Sonic 1 and skip to the Labyrinth Zone act 1. I made it to act 2, and I'm pretty sure I was close to the end of the level, then I died and got a game over.
The Labyrinth Zone is just filled with too many cheap deaths. That, combined with all the slow moving elements (moving slow under water, this stupid thing, etc) just makes it very annoying to play.

You can do it! I don't know if it helps, but I make sure to stop at every air bubble to refuel no matter what, even if I just grabbed one two seconds before -- and if the timing is off for whatever's coming next, I'll sit with those bubbles until I'm sure I've got a path to immediately move after grabbing one. For the spikey dudes, if you duck, their spikes will pass harmlessly over you. (I can't remember if rolling has the same effect.) And for the boss, try to ignore the water. If you focus on avoiding the lances and slowly time those jumps, you'll make it out in time.
If you're motivated to play through the whole game, stocking up a bunch of lives before this point will really take the pressure off.
The other tip is not to panic. I haven't played through Sonic 1 in a few years, but if it's any consolation I'm fairly sure that there's no "being comfortable" all the way through Labyrinth Zone. We all know what that drowning song does to us, and you're probably going to hear it a lot here. Just because you're stressed doesn't mean you're not doing well. You got this!
And while we're sharing, I've never been able to get all the Chaos Emeralds in Sonic 1. Something about those Special Stages confounds me I guess.
