You know what I'm talking about, an old raggedy notebook you'd always keep your cheats stored away in.
For me it was the pleasure whenever friends shared cheats from Golden Eye, Diddy Kong Racing, or other N64 games. Hints went in there as well.
In fact, I still have my 14 year old book lying around
I have many, many composition notebooks full of gaming notes. Routes in shmups and scoring tricks/goals, Pokemon breeding processes and type strengths/weaknesses of my team, all that fun stuff.
Sadly, any from long ago are lost. I'll hang onto these this time though.

It is a bit sad that my handwriting hasn't improved since 1985. Actually, it's probably gotten worse.
I always have a notebook handy to jot down jokes, and a lot of them are illegible. Sometimes I don't even write down the joke, but just words. Something compells me to wake up in the middle of the night to write "Google Maps" for some reason.
I literally had an entire binder full of notes, diagrams and maps for Riven on PS1. That thing was awesome. I always took notes for games like Silent Hill to help me work out the puzzles. Kids these days just have to look it up on the internet if they get stuck

Hopefully somewhere I still have my personal "Map to Metroid" around. It's a bunch of notebook paper, stapled three times, and I even traced the cover art for the first page. It's a mess of a map, but I put a LOT of time into trying to make it artsy. My execution was terrible, put my map would show you where to find every E tank.
I never really wrote down any notes when playing games. Had a couple Tips & Tricks magazines which I kept around so I didn't really need to write the codes down, and at least where games are involved I have a pretty solid memory lol.
I did have baggies full of RPG style board games I made when I was going to day care though, but I think those are all gone now though . Used to spend days planning out the boards and hand drawing monster and character cards.
Nope. Very few games I take notes for. I make mental notes of lots of things however, cheats and what not. I can even remember passwords for some games I played as a child.

That's the level select code for The Lost World on PlayStation, for instance.
I hardly ever take notes (and I don't usually need to since most of my games are fairly new :/) but I did take notes for hybridizing in Harvest Moon: Another Wonderful Life... Mostly just for what veges/fruits make what.
Oh and recently I made a dungeon map for Shining in the Darkness... That was pretty fun
