Super Castlevania V (Dracula X or so fort doesn't count..they stopped numbering). Its like Mega Man X....it's a whole different series/story.

A real sequel to Cybernator/Assault Suits Valken. Metal Warriors doesn't count, though should have also had a sequel.
The aforementioned Run Saber could've established more of an identity beyond "Strider Clone" in a more fleshed out sequel. Hagane as well, just because the SNES needed more action games like this.
Axelay and Sunset Riders, enough said.

Yes the Japanese one is almost always better, but I never gave it any thought that the boxart of the NA version had very little to do with the actual game.
And Slapstick was a great title for the game.
