Well, I killed the grim reaper...only to have his scythes remain on the screen after his death and then kill me. Seems fair to me.

Ninja Gaiden and Castlevania are both easy to the point of it feeling like cheating if you use the proper weapons.
In the first stage of Castlevania, you can get level 3 Fire Bombs. Keep them by not grabbing any other weapon and you can use them all the way until the ending credits. Bosses such as the Grim Reaper and Frankenstien will literally be unable to move under constant Fire Bombs.
The super weapon in Ninja Gaiden is the spin slash. Most bosses die in one hit to it, including the first form of the final boss. Zzzzzzzip, boom. Just as with Castlevania, memorize the item locations and skip anything other than spin slash. It also makes 6-3 a breeze, if you gather enough power to use it frequently.
As for the game with the cheapest deaths, I vote for Startropics. It has several trial-and-error jumps, where you won't know that it kills you until you've done it at least once. It also has negative 1ups, where you actually lose a life instead of gaining.
My memory of cheapest and worst death is from Baldur's Gate. Along the coast you meet a woman. A certain series of dialog choices leads to her kissing you and results in instant death for the character. If that character happens to be your player-created character, you must reload from a previous save. I lost a lot of time due to that death since I tried to play a little realistically and only saved when resting.

Well the game I have to offer up does not have death, but knockouts. Freakin Punch out on NES. I can't tell you how many times I am just dominating and then out comes the computer with the unavoidable fast lefts and right which usually end up in me getting knocked down. You really have almost no choice but to take the hits. Ah well, I can't stop playing because it's pretty fun and addicting. At least I can get past Glass Joe though!
