Killer7 and the No More Heroes games are definitely among the craziest I've seen. Everything by Suda51/Grasshopper Manufacture is very weird to say the least, I haven't played every one of their games though. Even the anime games are usually based on strange series. The upcoming Killer Is Dead looks to be pretty whacky too.

I was gonna say Suda and his team I wouldnt even know where to start. There are tons of weird 3DO games too like Pataank and Blade Force.
If you can see the future while remembering the past, you may just have control of the present.

There are so many ways to define weirdest in this case. There are many themes, gameplay mechanics, and other things that can be weird in an otherwise normal game. There are three games that really stand out in my mind as very strange that I have played.
LSD on the PS1 is probably the weirdest in theme and environments. It's mind bending, confusing, and entertaining. It's pretty basic and crude at this point, but, at the same time there is nothing quite like it.
I'd like to also say Hotline Miami. The game itself isn't that weird. The feeling it gives you while playing is weird. They do a great job of mixing feelings of confusion with the rush of a perfectly executed killing spree. The sickening swaying of the camera and the bright neon lights just perfect it. So while the mechanics of the game aren't weird, the way it made me feel certainly was. I know I'm not the only one that was both unsettled and enthralled by it.
Intelligent Qube on the PS1 is weird in an amusing way. It's probably the strangest puzzle game you'll ever play. Strange mechanics and a strange presentation. I like it a lot. It's very difficult though.

I'm Playing No More Heroes on PS3 now LOVE IT.
