Ico and Colossus are hardly unknown or underrated. Same goes for Beyond Good & Evil. The gaming press raved about all these games and gave them high reviews. The sales just weren't as amazing as they should have been at launch. i quite liked extreme g 3, its fast and furious and seems to have dissapeared without a trace.
for things a little more obsure titles at uni we had many a drunken blast with the original jap ape escape where you havce to chase the little apes arround and hoover off their trousers and put them into a giant washing machine..... well its obscure.. not sure its a undiscovered gem tho

I'll second those. Extreme G3 is quite good -- it fixed everything that was wrong in the series. Burnout 3 - Not an unheard of gem, but a great game.
Disaster Report - Although I got stuck and never finished it, it was quite interesting. You wouldn't think a guy could die from thirst so quickly.
Indigo Prophecy - I have it on the XBox. It's a pretty cool game with crap controls. If they make a sequel with better controls I'll snap it up in a heartbeat.
Magic Pengel - If you like the thought of drawing your own monsters and then battling them in an arena, it's pretty cool. The combat is modified rock/paper/scissors.
The Mark of Kri - May not be all that unheard of, seeing as it got a sequel, but if you haven't heard of it you should check it out. It's quite old so you should be able to find it cheap.
Metal Arms - Again, I got this one on the XBox. However it's quite a lot of fun.
Psychonauts - Another one that has actually gotten a lot of great word-of-mouth and may not be considered by many as an "unheard of" gem, but it's a fantastic game that you shouldn't miss.
Shadow of Destiny - It's a blast unraveling this one, although it can be frustrating as hell. And the controls aren't optimum. It might be hard to find.
The Simpsons: Hit & Run - GTA Lite. It has its flaws, but it's still a fun game.
Barbie Horse Adventures: Wild Horse Rescue - Ha. Just kidding.

I reccomend pretty much any of the PS2 Gundam games except that SEED one or Journey to Jaburo. They're kind of hard to find now, and they actually didn't get too much press and I think most pro reviewers bash them simply because they're games based off of an anime, the proffesional reviewing community tends to spurn anime games even if they're good or bad, one example being when Game Informer gave Budokai 3 for PS2 a 5. I didn't really understand that at all. Especially after they gave the second game such a high score.
While it's true that most anime games aren't exactly great there are a few out there that are pretty good. Most of the Gundam games being those imo.
I also reccomend any Zone of the Enders games, Blood Omen II, Soul Reaver II, Legacy of Kain Defiance, and R-Type Final.
While the Legacy of Kain games and ZoE are usually rated pretty well they really don't get too much attention.
