- Grinding and leveling up and then running into some bastard like a Marlboro that wipes out your entire party making you lose all the time on grinding. I quit playing one of the FF's for like a month I got so pissed about this.

- Pointless sidequests. Running all over the place passing letters or collecting trivial items, or having to talk two people into hooking up because they are too chickenshit to talk to each other.

- I've never been able to get into games like Resident Evil, Silent Hill, etc because I hate the the controls for walking/running.

- crappy controls

- Bad hit detection

- platform games where the character has crappy jumping ability

- Puzzles in games that are not puzzle games. I like a good puzzle game... I don't like having to figure out some crap puzzle in an RPG or Zelda like game that is usually based on some arcane piece of information obtained much earlier. This is about the only time I consult an FAQ on these type of games.... which brings me to,

- Not having a clue on where to go next to progress in the game (also one of the rare times I consult a guide to get through a game.

And I second most everything else that has been mentioned, but especially bad cameras.

Vague quest objectives. I hate going on quests in Skyrim and not being able to advance because I'm wasting an hour running around some town looking for some NPC I have to talk to. JUST TELL ME WHERE THEY ARE!

I loved that! felt like good old survival horror, kinda reminded me of silent hill 2. That difficulty spike at the end of dead space 2 was pretty crazy though

Fixed camera angles and tank controls are essential for survival horror games like that.

Three words: pen and paper. RPG games are based on pen and paper RPGs and many hardcore/oldschool RPGs require you to do things like make maps, read instruction manuals (*gasp*) and make notes to a piece of paper. It's the same thing with Survival Horror games like Silent Hill too.
My WTB thread (Sega CD/Saturn games)
Also looking to buy: Ys III (TG-16 CD), Shadowrun (Genesis) Hori N64 mini pad and Slayer (3DO) in long box/just the long box

Games that make you fight every single boss (that you already defeated) again as you approach the end of the game.

Fighting game bosses that are totally unbalanced/seem to be from a different game. Example: most of Capcom's VS games as well as most SNK games.

Unskippable cutscenes.
Mostly harmless
