Dialogue.
Sewer levels.
Escort missions.
Required internet connections for single player games.
Cheating AI: rubber banding in racing games, AI that knows where resources and etc are that shouldn't be visible to them (Civ III was pretty bad for this), games where when difficulty is increased, instead of making the AI less stupid or just giving them more health, they just make them cheaper.
Lack of item switch. After Super Smash Bros., there's no reason for there not to be an item switch in like every fucking game. It's a travesty that Mario Kart still doesn't have one.

The number one most annoying thing for me is cutscenes you can't skip. Obviously cutscenes are important to story driven games, but many games have pointless, unimportant cutscenes that you can not skip. Here's some more annoying things:
Ice levels thrown in just for variety:
Most ice levels are usually just tossed into games for a bit of variety, and you pretty much always know what to expect: rage. Slippery, slippery, slippery. One of my favorite games ever unfortunately had a few very annoying ice levels, that being Bomberman Hero. Granted, some games do ice levels very well, such as Mario Galaxy with the ice skating, and Ristar how once you start sliding you can't stop until you hit a barrier.
Too much dialogue:
My favorite game of all time is unfortunately notorious for this: Fallout 3. God damnit man, I want to play a game, not read a book. Being a huge comic book nerd, I can simply pull out a Spiderman story if I'm in the mood to read. I don't need all this ridiculous dialogue in the game, JUST LET ME PLAY!
EA servers:
Is any description really needed? They sucked years ago, they suck now. Constantly down, and when they are up they are often laggy.
Forced difficulty:
A game I just got recently suffers from this a ton. The game is Dragon's Dogma. It is a great game, but it is really just unfairly hard. I mean, after a ton of grinding and completing a bunch of side quests, I still can't progress through like the 3rd main quest in the game, despite having had it for about five days now. You know why? A god damn dragon, that's why. The game tries to make itself last longer by just pushing difficult enemies on you too fast. It's ridiculous, and way too many games do this. At least have different difficulties, but nope, not an option. On the other hand, another game I got recently is also a ton of fun, but this time way too easy. Prototype 2, and after having played through it on impossible difficulty, I can honestly say it is the easiest game I have ever played. Yes, easier than Dora: Journey to the Purple Planet.
Those are just a few things. Having played hundreds of games, like many of you here, the list could go on for eternity, but those are probably the worst offenders. Oh yeah, and bad cameras.
