I'm not sure, but I think an MGS game did that.

First thing I thought of was the ending you get in Breath of Fire II if you refuse to go into the last dungeon: The funny thing about that is that most yes or no answers in old school console games just force you to pick the right answer. Usually it's fun to pick the wrong one and see what the game does before making you pick the right one. In this case, tho, it's fuckin game over, you suck, the world is dead thanks to you.

For a more recent game, SMT: Devil Survivor's "Mind link disconnected" game over screen is pretty jarring and downright rad:
