Astal question

Not sure if anyone here knows. But I was curious how you get extra lives in Astal on the Saturn. The (only) faq online says that you can get one on level 5, The River of Dreams when the dragon you are riding stops. Now, sometimes I get an extra life here and sometimes I don't. All times I have full karma. But also I noticed that I can get an extra life earlier when I have full karma sometimes as well. Any body know? or have any ideas?
