I have a US Turbo Duo, I've researched at length game type compatibility, but I never remember hearing anything about controllers...
1.) Does the Turbo Duo use the same controller connector as the regular TG-16?
2.) Is The Turbo Tap compatible with the Duo?
3.) Are Japanese controllers and accessories compatible with the US version of the Turbo Duo?
4.) Any place other than Ebay to find an extension cord, is it possible to make one?

No, the TG-16 uses large plugs. The US Duo and all Japanese hardware uses the small plug. They have the same number of pins and can be adapted.
2.) Is The Turbo Tap compatible with the Duo?
As above, the plugs are different, but they are electrically compatible.

As above, for all systems that use the small plug the controllers and taps are compatible.

I'm sure there are extensions floating around other places too. I made a couple of 6 foot extensions for my US pads. Just need to get a DIN plug and socket and a cable with enough wires in it.
