Anybody know of a company that produces custom game boxes?

Anybody know of a company that produces custom game boxes? Preferably a place that can make a custom PC game box (that can hold a CD as well as a booklet). I know there are places that produce CD's for bands with booklets, but I'm looking for something with a box and booklet. If not, is there a place that makes retro game boxes?

How big would the booklet be? Small comic or regular manual size?

I'd say regular manual size. I'm thinking like the size of a Genesis or SNES game.
