The first game that comes to mind is Naughty Bear. There's some really sick and sinister shit going on in the game yet it got away with only a "Teen" rating.

"Though the art style is overtly colorful and 'cartoony,' players have the ability to finish enemies off (e.g., slamming heads with car doors, breaking necks, stomping, impaling) and drive teddy bears to commit suicide (e.g., self-stabbing, shots to the head). Throughout the game, the narrator provides guidance and encouragement with the intent to be humorous; for example, 'Awesome Electrocution,' 'Defluffication,' and 'When a bear becomes insane, you can run up close to him and perform an outrageously naughty move . . .'
I'll be damned if that's not a serial killer or "lunatic" simulator aimed at children.

Considering how many kids watch Adult Swim and Comedy Central these days, I don't think it's unreasonable to say the game was intended to sell to kids to at least a moderate extent. If you're a 13 year old boy who likes violent games & movies it just might!
