Like many others, I've learned to stay away from most 3rd party memory cards.
Lost the 1st 10 hours of Final Fantasy VII on my 1st play through (Performance brand). For some reason it didn't bug me that much (caught up to it in 7 hours).

Lost a bunch of saves on my Dreamcast once (--edit--on a Performance card as well, not Interact). But I had beaten most of the games anyway.

Lost my Morrowind and Phantasy Star Online saves (not too far actually) on my Xbox, when the system died. I guess I could still swap out the hard drive, but that's too much a pain for lazy 'ole me.

One day I forgot to put my memory cart in after turning on my Sega CD combo, and so like an idiot I inserted the cart WITH THE SYSTEM ON! I vowed never to play Dune again. Also lost a couple battles in Shining Force CD, but that wasn't too bad.

And then there's the times I screw up games saves and save states playing on emulators... but I only have myself to blame there too.

I've had others, but those are the ones that stand out. You'd think I'd get sick of it, but I still think most save losses are preventable.

Oh, and someone mentioned the Soul Calibur III save problems. I used to experience this too. But you can generally avoid save corruption if you know what causes it (basically turn off autosave and don't delete anything saved before it on the Memory Card; I just make it the 1st game on the card).
Not saying that corruption isn't still possible, but I've yet to run into problems after following advice shown here:

And then I back up a good save on my PS3, just in case. Of course, I still have to re-unlock everything when using a Virtual Memory card with OpenPS2 Loader (trying to transfer the save corrupts it), but I'm okay with it.

I remember Sony released a demo disc for the PS2 that erased everyone's memory cards. I was one of the victims, but Sony was nice enough to give away a single *budget* title to everyone affected. That's how I got Sly 2, they also punched a hole in the box so I couldn't resell it. :P

Reading through these made me think of another situation. Me and my girlfriend used to (and still do) play the crap out of Phantasy Star Online. I had my Viridia HuMar up to lvl 124. I was actually 1 lvl away from being able to equip this badass sheild (stalwart sheild or something like that)

Well, I went to work, and my gf cleaned the house. The memory card was never seen again. She thinks that she may have thrown it away, but I have high hopes that someday I'll find it laying around.

Oh well, my section ID sucked anyways.

4th gen was the 16-bit era, The only non-adventure/RPG titles with saves I could think of would be Kirby's Dreamland 3, Sonic 3, and Super Mario World.

This topic reminds me of my Sega Channel rage. Sega Channel used to switch out its games every month (some would stay for another month or so at times) and your saves I guess would be remote. I had been playing Shining Force II for a couple months and I had to be a third of the way through the game... well one night at midnight the games changed over... and Shining Force II was not on that list. My entire progress was gone for good. I almost broke my genesis at the time.

Just a few years ago I went back and beat it. Resolution felt good.

Just ran into a glitch in Dragon Age II that will prevent me from being able to recruit someone... I'll have to reload an earlier save and lose nearly three hours or play time in order to fix it. :x

I have a torrid history with Final Fantasy VII. I have never gotten passed the second disc due to the computer, psx, ps2, or ps3, screwing up in some way. Ive tried it most every way you could imagine and everytime something gets screwed up: be it the memory card, the laser, the harddrive, etc.
