I thought the chase sequence in Metal Gear Solid 3 was amazing till I played 4. The motorcycle chase in that one just blew me away. It still does.

One of my favorite chase scenes is at the end of the first chapter of Half Life 2. You don't have a weapon yet and you are just making your way out of City 17 with the Combine in pursuit. You are up on the rooftops and jumping away frantically. The path is ultimately fairly linear, but it feels like a harrowing escape.
The most adrenaline pumping chase scene though was the escape from the water monster in the sewers of Amnesia: The Dark Descent. My heart was pumping and I was freaking out the whole time. Here's someone playing through it, though it's much more scary to play it than it is to watch:

Mirrors Edge had several 'oh crap oh crap oh crap' awesome moments. I also agree with the HL2 one.
It was more of a race against time, but the push into the reactor building near the end of the first Stalker before the blowout was pretty intense trying not to get shot too much and finding exactly where the door was located. I know I've played through another recently, but I can't remember what it was.

When I was a kid, I spent AGES trying to beat the Just Cant Wait To Be King level, the final puzzle where you roar at monkeys took me ages to solve. Haha, this sounds dumb, but one "chase" scene I remember is in the original Lolo. I can't remember exactly which level and can't find it on youtube, but in one of the later levels it's not so much of a puzzle as it is running away from one of those stupid aardvarks until you can open the treasure chest. Kept my heart a thumpin' haha.
