Lots of good mentions, here are a few I can think of that arent in the thread yet.
Full Throttle- An old PC title that is sort of an interactive cartoon, with a lot of puzzles to solve.
Eternal Darkness- For the GC, has some of the most unique breaking the fourth wall moments.
Oddworld Strangers Wrath- An Xbox action 1st/3rd person shooter with a great character dynamic.
*Odin Sphere- Is awesome if you like 2d combat games, the art is amazing.

You may have heard of these but here's what comes to mind... (may also be some repeats):
Panic! for Sega CD is pretty darn strange. It consists of little more than going "ooh - what does this button do?" but it is amusing to see all the wacky results.
I always thought Monster Party for NES was extremely weird, and it probably scarred me as a child. The first level's transformation, the bloody in-between levels screen, the final boss and ending... also, one of the boss fights is a series of rogue deep-fried food (name another game where you fight fried shrimp and onion rings).
Muscle March for WiiWare looks really fruity and fun.
E.V.O. for SNES is unlike anything I've ever played. You gradually evolve your animal through the course of the game, starting out as a fish, and eventually moving onto land, killing anything that gets in your way. I've never managed to get very far, but it has a unique concept and challenging gameplay that rewards curiosity and experimentation.
Earthbound, also for SNES, is very strange and has a quirky sense of humor... very well known, you probably know that one.
I think Bangai-O for Dreamcast is pretty weird, mostly for the goofy, intentionally poorly translated story bits.
Space Funeral is a very unusual freeware RPG featuring some bizarre/awesome 70s psychedelic music, like Les Rallizes Denudes... who happen to be one of my favorite Japanese bands as it is... I was one of the few who knew of them before seeing the game. I haven't actually played it yet, though, but its weirdness alone has a hold on my interest.
There was a Japanese PS1 puzzle game called Tsumu where you played as a hamster operating a forklift, with the objective being to construct some nation's flag out of TV sets... or something like that. It was on some Playstation Underground demo disc, or some such, but good luck finding anything on that. I thought it was pretty fun, back when I played it.
Roogoo is a fairly inventive puzzle game for XBLA, I don't hear it mentioned in a lot of places. I find it enjoyable, if a bit kiddie themed.

Ecco the Dolphin is probably one of the weirdest gaming experiences I've ever been through. Looking at the covers and playing the game at the start, you think it's some cutsey undersea adventure or something, until that vortex sucks up your friends and pod. Then the game becomes something else, as you get further the game visually progresses this overwhelmingly daunting plot... there's not much to be said without spoiling the experience, but anyone who's made it through the whole thing probably understands. It's way out there.

I tried playing it once. Couldn't get anywhere.
