The versions: PS2 Black Label, PC CD-Rom, and a PC DVD-Rom.
I can confirm that the PS2 and PC CD-Rom versions both have the soundtrack included on an extra disc. I cannot confirm the PC DVD-Rom version until I buy one...
I've yet to find a PS2 label that doesn't say "Includes Soundtrack". I'll be on the lookout though. There may exist a version that didn't include it.
However, I just came across this as I was typing up this post. From the Silent Hill wiki:
This may mean a search for a soundtrack-less copy is futile if it is referring to all U.S. releases.The American release came with an extra soundtrack CD, titled "Silent Hill 3 Official Soundtrack" (containing 25 tracks), a first in the series.
edit: By "all U.S. releases" I mean all U.S. PS2 prints. Surely they made more than one print-run of the game.
