good call on the mohawk and headphone jack, I've had that game laying around for years and had never played it, pulled it out last night and was very impressed.
I hesitated mentioning Katamari, because it is one of those games that hit mainstream and it just feels wierd to call it alternative at that point. You might as well add The Sims to that list then.
I dont know how many people have played/heard of Art Dink's Tail of the Sun [PS1]. Now I know sandbox style games have become quite popular but at the time of its release it was very unique in style. Actually any Art Dink game probably qualifies, might want to check out No One Can Stop Mr. Domino [PS1] for something very different.
You'll probably find alot of interesting gems in the list of system release day games, Fantavision [PS2] comes to mind.
Also I think Disaster Report [PS2] would qualify and possibly Incredible Crisis [PS1]
Wasn't there a VC Hampster game for the sega cd or did that not get released, cant remember
Fave Alternative games? TJ&E, Vib Ribbon, Rez etc...

This game has been very intriguing to me. I want to check it out whenever it's finally released.

Stretch Panic. Sure it's 3d run-arround, but just as crazy as TJ&E.
Snatcher. It does fit the genre for Digital Comic, but that's a pretty "alternative" type of game IMO.
There was that sound game for SS, where you didn't get to see anything, only hear whats going on (not a big import title). The guy who did D and Enemy Zero created it. I liked EZ a lot, but the survival-horror type of game has been played out.
Art Truck Battle and the train simulators on PS1 were at heart a driving game, but just not a car. ATB had awesome traditional japanese music.
