I would say Eraserhead or Inland Empire. They are really uncomfortable, and David Lynch is a master at crafting nightmare-scapes.

Tale of Two Sisters- Korean Psychological/ GHost horror mind-fuck fest- truly frightening
Session 9- story about some guys renovating an abandoned insane asylum and the shit that goes wrong inside- pretty freaky
Frontier(s)- a french take on texas chainsaw- very gory and frightening
Inside- another french movie about a woman trying to steal a pregnant woman's child from the womb
Psycho- Hitchcock masterpiece, gives me goose-pimples every time

Big second for Tale of Two Sisters. It's a really great movie, and worth reading explanations and watching it multiple times to really get whats going on.
I'l add Candyman, and Black Swan. Neither are outright scary at all, but it's the sort of visceral semi plausible stuff that really sticks in your head.

The Amityville Horror book was incredibly scary, but for movies, I'd have to go with The Exorcist.

I know it was considered extremely scary at the time, but I found The Exorcist to be hilarious, especially the scene where the girl stabs her groin with the crucifix.

I don't find many horror movies to be all that scary, mainly because I don't really believe in the supernatural, so I don't get freaked out by that stuff like other people do unless I let myself buy into the fictional world.
My top 3 scary movies are The Shining, The Blair Witch Project, and The Descent.
The Shining I have watched many times. It still gets to me. I love how it's open to interpretation as to whether the place is haunted or Jack is just losing his mind. This scene still freaks me out the most because it comes out of nowhere, is never explained, and just leaves you going WTF? It's almost funny seen out of context, but in the movie it just adds to the insanity of what is happening near the end.
The Blair Witch Project was really scary when I saw it in theaters. I've never felt compelled to watch it again though because the shitty camera gimmick really only works once. It did feel like something real though because of how it was shot.
The Descent is frightening mainly because it is in a cave. I got stuck in a cave briefly once while in a passage about 10 inches tall. I think being scarred by that real life experience makes the claustrophobia of this movie all the more terrifying. Add some monster mutant things to the mix and its a real nightmare.

Both of these are up there on my lists as well- I could never imagine being stuck in a cave- so kudos to you for having some big balls! Also, If you liked BWP- you should check out the movie, The Last Broadcast, it is one of the films that kickstarted that whole BWP sort of horror movie, its really low budget, and I would be hard pressed to say that it is a good movie, but worth checking out for the history of it

As a 5 year old I saw Childs Play which absolutely scared the crap out of me and continued to scare the crap out of me for a few years afterwards.

I loved the Exorcism Of Emily Rose and Paranormal Activity. Whats weird is they were both scary the first viewing, but the second viewing while still good wasn't as scary. However The Blair Witch Project is scary no matter how many times I watch it.
