For me it's Intelligent Qube. It's such a brilliant and charming puzzler for the PS1. I love double dropping bombs at the right moment and getting epic points!
True Genius! 15000!
The orchestra music and the announcer commanding at you "Again!" when a row of blocks tramples you and 3 or 4 lines of your play area fall off into the void.
CLASSIC!
For those of you who love this game as much as me, check out Kurushi Final. It's basically the sequel to IQ. I believe there is a eurupean version called IQ Final but the Japanese version is pretty much all in english.
Runners up:
Shadow of the Collosus (PS2)
Image Fight II (PC Engine)
Cave Story (PC)
Megaman X (SNES)

I've lost count of the number of times I've completed it, but what I love most about it is I was still finding new experiences in the game years later. Even now, whilst I've experienced nearly everything, there's probably still stuff I've yet to see. The same applies to the sequel too, but I have such fond memories of the first game.
I play through Shenmue at least once every year and I still love it.
Same. Every Christmas if I can. If I don't manage to then the following summer I make sure I atleast spend some time in the slot machines.

I'm 29, never gotten rid of any console I've owned, going all the way back to the Mattel Intelevision II, which was my first console. I've sold maybe a dozen or so games over the years that I really didn't care for, but have kept almost every game I've ever bought over the years. I own hundreds of games, yet, even I have a few favorites that I play through again and again, at least once a year.
Tetris (Often, and on whatever console I am currently playing, I have a version of it for almost every console I've ever owned.) Probably close to once or twice a month actually.
Sonic 3 & Knuckles - Be it Xbox Live Arcade, or a compilation disc on any number of consoles, This was a favorite growing up, and I play through it at least once a year, sometimes twice. I rarely get the Genesis out of the closet for it though, since I have it for many other consoles.
MegaMan (Original Series 1-10) - My favorite game series ever, I play through every game in the series at least once annually, in a marathon run usually, and then occasional play throughs of certain ones any other time of year as I feel the urge to play them.
The Legend of Zelda III: A Link to the Past - My favorite Zelda game ever (Just BARELY above Ocarina of Time, which is a VERY close second) I know I've played this game through all the way at least once a year, every year, since it was released. Ocarina, by comparison, usually gets played through every 1.5 years.
Chrono Trigger - Favorite RPG ever, it gets played through at least once every year or every other year. That NDS version that came out a couple years back was pure gold, after having played it to death on the SNES over the years, having some new time portals and some expanded story segments that made sense and explained everything was amazing.
There are many other favorite games that probably get played through at least every two years or three years, but these listed above are probably my overall favorites that I know for a fact get played close to once a year every year. There are a lot of modern games that I play frequently now, but only time will tell if they stand the test of time, and I'll still be playing them ten years from now or not.

I'll third that. Every holiday season i try to play through the first one. It never really bores me for some reason.
Well, I can't say "Shenmue" because I'm playing it for the first time in these days, but I'm sure that I'll play it again and again, it's just awesome.

Yup.
There aren't any games I play at certain times as a general rule, but I often have a Koei phase and then have a long break from them when I've had enough of them for the time being.
