Every Christmas I play Christmas NiGHTS. It is just one of my christmas traditions. I have also been playind Diablo 2 every now and again for the past 3 years. I also play a little sonic every so often.

Animal Crossing (GameCube)

I love the atmosphere, setup of this game, but not too often
I've already over-played it a couple of times, now I take it easy on it

Zelda series

I play through the Zelda games, every year... takes me about one year to finish each, before I continue to the next one (I have 9 of them)

S.T.A.L.K.E.R. - Shadow of Chernobyl (play through the main-story with mods, about once every month, since release)

Quake 1 - to remind me of the golden era

Gothic 1 - about once a year, since release (or at least since 2005)

Morrowind/Oblivion - come into a frenzy, where I mod the living daylights out of both games, set it up perfectly, then play it for a few hours/half-year and leave it since I've perfected them (these games, modding is the best part for me)

Rags to Riches (Commodore 64) - because
C-64 will never die

Mine happens to be on Dreamcast as well. I always feel the need to play Virtua Tennis every once and a while.

The Legend of Zelda: Ocarina of Time (I have played the pants off this game but not always yearly)

Marvel: Ultimate Alliance

And I have a tradition of beating S3&K every new years eve before midnight.

EDIT: Almost forgot, Turtles in Time for SNES is pretty much monthly too.
