Yeah, basically any underwater level of a game where you have limited air freaks me out. As a kid, I never dared to play the Jolly Rodger Bay or Dire Dire Docks levels; it was way to tense. And that giant eel in the former didn't help very much, either.

Game moments that I have found scary at some point. Note that I was young for some of these:

NES:

Super Mario Bros.: The first few times I tried to play, this game honestly really stressed me out. I was very young, and dying in games really bothered me a lot. This meant that I usually didn't take any steps to actually complete a game or get a high score for a while after I started playing.

Metroid- The first time I actually made it into Tourian and encountered a Metroid. They were a lot quicker than any enemy you encounter earlier in the game.

Final Fantasy- The Marsh Cave

I also seem to remember getting a little bit scared in the last labyrinth in Dragon Warrior while headed to fight the DragonLord.

Sega Genesis:

Sonic running out of air.

SNES:

Super Metroid's haunted ship was a little spooky to me the first time I played it.

Alien 3 was pretty creepy, I thought.

N64:

Ocarina of Time: The zombie things were really spooky to me the first time I played. Also, Gannon's final form was rather scary looking. When my wife saw me playing, she said it was going to give her nightmares.

Mario 64 and Conker's Bad Fur Day: I forget who mentioned it earlier, but I also hate dying by drowning in video games. Both of the games I mention here also feature giant sea monsters, which are also a phobia of mine.

PC:

Monkey Island 2: LeChuck's Revenge: The last confrontation with LeChuck is a little scare right at first, but ceases to be when you realize that nothing really bad can happen to you.

Maniac Mansion: Any time you have the power off or the pool drained and know that you have to be fast to keep the mansion from exploding.

Gamecube:

Geist: I'm not sure if this one counts. I'm not sure if it's really a horror game or not, since it involves spirits, possession, and demons. It definitely has spooky bits, though.

Other:

Oh yeah, also Missile Command, Asteroids, and Berzerk all scared me at least a little when I was little. All of those old games where death is a certainty are really, really bleak, imo.

I always classified Half-Life 2 as somewhat of a horror game. Although foremost an FPS, it just has too many scary moments to not fall into the genre in some way.

It's not that impressive now, but back when the game originally came out and I played through this area for the first time, this scared the crap out of me:

I can't say I've ever been scared by a game, and I've played several horror games too. I just don't find them scary. The things that have a better chance of scaring me are the ones that try to make it seem possible, and that it could happen to you. Most of those kinds of stories are in books, though.
