Uzu Keobukseon is by no means a hidden gem. Its "hidden" but its certainly not a gem, especially compared to all the other great Japanese imports that could take its place on that list. I'm actually not too familiar with Japanese imports, but here is a thread I remember from Sega-16: http://www.sega-16.com/forum/showthread.php?t=9411 - It would be up to someone else to decide if they're hidden.

What about Gimmick for the Famicom? Someone posted a really great link (In the Nintendo section) showing a full play-through of the game that was very well annotated.

Seconding Umihara Kawase as a good import for SNES and Umihara Kawase Shun for the PS1. For Xbox ones, only one I can think of is Metal Wolf Chaos.
