Kid Chameleon? It has over 100 levels and no save feature. Although I guess that could be considered cheap. Has anyone ever beat this game without cheating?

Comix Zone was so hard I give up quickly after getting frustrated. I find The Revenge of Shinobi really hard to play, I didn't beat it but I think I got to the 6th or 7th level until I couldn't continue anymore.

I'd say Earthworm Jim 1 and Mickey Mania can be rather difficult particularly the last few levels. As far as I remember they weren't cheap but might have one or two frustrating sequences (like navigating the submarine in Earthworm Jim)
