Pretty much everything of mine made by Sony (except for the cheep headphones) just die on me. My B&O stereo has outlasted all of there products combined and it's an antique (a lot of people seem to think that Bang & Olusen stuff is of poor quality).
Most reliable (IMO)
anything cartridge based especially Gameboy
my worst
PS2 (overheats while playing ps1 games)
Sega CD (my model 1 is possessed, it works when it wants to)
which console is the most reliable... which is the worst.
Re: which console is the most reliable... which is the worst.
I forgot to post my least reliable systems:
Dreamcast -- Although I love the system, didn't pulling out a controller cable while the system is running and then plugging it back in cause problems? I seem to remember more than one controller port being fried because of this, even though everything else seemed well designed.
PS2 -- My brother had an early model PS2. Then a mid-life model. Then a later model. I know the games were good, but having to replace the system twice would have upset me quite a bit.
XBox 360 -- My cousin wanted to sell me on the console, so was telling stories about how great Microsoft is to deal with when the system fails. That is not a selling point. I understand that crap happens... but it shouldn't happen more than 50% of the time.

I would say my Gamecube has been the most reliable. As far as worst goes, definitely my 360 or should I say... "360s". I love the console to death, and death comes quickly.

the gamecube was built like a beast. my only problem ever was that the laser lens went out on me. just replaced it and was good to go!

Re: which console is the most reliable... which is the worst.
I've gone through 4 genesis, first one died because I spilt soda in it, 2nd one died after 12 years of constant playing and rough handling. My nomad still worked but was wonky, and my 4th gen is still kickin. So sega hardware is fairly reliable in my opinion outside those events. (which were mostly problematic due to my actions.)
I've had to send my DS in for repairs 5 times now, least reliable nintendo I've had now. They no longer make them out of Nintendium. Other than that I think the PS2 is the most unreliable piece of hardware there is.

Re: which console is the most reliable... which is the worst.
I have never had a Sega or Nintendo system break on me, although my brothers GCN once fried. I have never had a Sony system break on me, except one that my brother spilled a Dr. Pepper on. I had a PS2 by then though, so I wasn't too upset.
However, Microsoft...
I have had too many broken MS systems for my tastes. They are worth it, but both the 360, and in my experience, even more so the original Xbox, break down far too often on me.

best: NES. same console for 23 years, never broken, all original.
worst: genesis. i have 4. 1 works consistently.

Re: which console is the most reliable... which is the worst
**bump** I've been thinking about this recently and feel like sharing my experience.
Most reliable: Sega Master System, Game Boy/Color, Gamecube (these systems have never given me problems)
Least reliable: 32X (I basically need to pull out/re-insert the flat cables inside it every time I want to play something.)
I have a few consoles that make me paranoid sometimes. My PS2 is acting like it might keel over any day, my Dreamcast's disc drive sounds like the apocalypse, my Sega CD seems like it might be relying on the 32X and something could go wrong from that, my original Xbox's harddrive might die, and I'm always afraid of damaging the touchscreen on my DS. These consoles don't actually cause PROBLEMS per se, but I'm always thinking that they're JUST ABOUT to start acting up.

To quote James Rolfe, the Commodore 64 "works when it feels like it."
