As far as where to get it custom cut... not sure but http://www.plexiglassonline.com/ seems to have custom cutting options though for what kind of price I'm not sure. Home Depot and Lowes don't offer that kind of service. Mounting would be pretty easy as long as you have a drill motor to make holes for screws in your stick casing, just have who ever cuts your plexi drill holes in each of the corners of the glass.

skate323k137 is helping me with that part -- we live less than an hour away from each other.

I may video it if he lets me so we can have a tutorial here :)

good question :D he probably just didnt want 8 buttons or wanted more space between the stick and buttons. I plan on only using 6 buttons and those are my reasons. Another thing about this particular mod is that he used clear coat spray paint and got a really nice result. :EDIT: no he didnt I was confused but it looks like he did O_O

There's a couple plastics supply places here on the west side of Michigan that can order just about anything. I could ask around and see if maybe there's some closer to wherever in the state you are.
As for mounting it most custom builders just use the buttons to hold it down, except that usually they're flush mounted. For that stick maybe some carriage bolts in the corners would do the job without looking too ugly, or maybe you can get more decorative looking ones at someplace like lowes.

Honestly, I find the all-black design very cool. I wouldn't mod it at all if it worked properly.

Most sticks are laid out so that the first 6 buttons resemble a JP cabinet and the last 6 resemble a US cab, so you can plug two buttons on either end depending on which layout you prefer.

The Mayflash is an exception to this ... the last 6 buttons resemble a JP cab. But check the Hori Real Arcade Pro sticks for a better example.

fyi I was in Lowes last night and they have sheets of lexan that look to be about the right size for around $5
