Possibly because it "ranks" how well you do, and thus "ranks" the difficulty based on this

Dynamic game difficulty balancing certainly transcends the shoot-em-up genre.
It seems however it was fans of the genre that started collectively referring to this notion as "rank."
i could be wrong.
What's the earliest example? Zanac?
