I'm getting ready to put together a contest for the site, but in preparation, I would like everybody to list games that they think are excellent, but haven't received the recognition they deserve (chose 1 to 10 games that haven't been listed yet).
Please post the titles below and once the contest kicks off, you will have an opportunity to tell why the games are deserving in order to have a chance at winning some great prizes.
Here's the list so far:
Bangai-O
Cannon Spike
Cosmic Smash
F355 Challenge
Fatal Fury - Mark of the Wolves
Elemental Gimmick Gear
Revolt
Typing of The Dead

Always gatta say
EGG. Otherwise I say Cannon Spike. Possibly Red Dog and Silver. I need to play those two more before I can be sure.

High resolution backgrounds. Mindless hack and slash always caters to me, lol. It had above average dialog. I haven't played in a couple years though. As with 400 other games, it's on my backlog.
Oh and I've only played for an hour, but 'Draconus: Cult of the Wyrm' is a fantastic looking and sounding game. Gameplay is stiff but manageable. Might become easier with practice. I've only played as the dude so far, but the voice acted dialog is fairly edgy and funny.

Seventh Cross Evolution - A evolution simulator in which you morph and create live structures. Poor graphics, but good gameplay.
Record of Lodoss War - Inspired from the anime series, the Dreamcast version plays out like Diablo - an over-head RPG with pre-rendered 3-D sprites. Fans of Runescape might enjoy.
Super Magnetic Neo - Slick Adventure / platform title with wonderful cute graphics and addictive gameplay, overlooked!
Bangai-O - Excellent platform/shump from Treasure
Project Justice - Impressive follow up from Capcom's Rival Schools (PS1) Japanese version includes a board game and character creator mode!
Cannon Spike - Reviving shump genre, Psykio teamed up with Capcom to create this short, but addictive platform/shump. Including 2-Player co-up!
Tech Romancer - Based on Old- Japanese mech culture, a shiny 3-D 1v1 fighter published by Capcom.
Giga Wing 2 - sequel to the Arcade / Saturn shump hit, featues a 2-player mode.

Shenmue wasn't exactly hidden -- it was one of Sega's headlining titles...
Anything else?

I would have to say Rez. I had not played this game until a couple of days ago, because it was never released in the US on the Dreamcast. I do not know if it is really a hidden gem or not, but it blew me away with its originality and game play.
