I have great ill-will toward X-Men on the NES for trying to pass itself off as a "game".
Also, Pit Fighter on the LYNX and SNES. SEGA's 16-bit version was strangely a better rendition, though it was merely "poor".
My scheduling skills have died of dysentery; I hope to visit at least on a monthly basis. Still, don't forget to tip your waitress.
I'd have to go with Jupiter Strike/Zeitgeist for PS1... I know I've said this many times, but it was seriously a terrible shooter... The music was awful as was graphics and the controls were a joke.. I'm mentioning this game because I played it quite a bit back in the day, because it was one of the first and only games I had on my PS1 for awhile..
Worst game ever is hard. Worst games I've ever played are definitely on the NES.

Those are standouts from my childhood as being just awful. I actually traded Silver Surfer to another kid for his granola bar -- every day for the whole school year.
