Echo Night Beyond on PS2. It's a first person adventure/horror/survival (no weapons in the game at all) game that takes place on the moon. Almost like Fatal Frame meets Metroid Prime.
Azure Dreams on PSX was one of my favorite RPGs as a kid, mixing tactical, turn based battles and pokemon/monster rancher elements.
Vib Ribbon on PSX. My friend loaned this to me years ago. If I remember correctly, the game's difficulty level was based on the type of music you played with...which one of the few PSX games I know of that supported audio CDs/'custom soundtracks'.
PLOK on SNES is one of my favorite platformers, with some of the best music of all time...not sure how known it is though.
Total Recall on NES is supposed to be one of the worst games ever made, but I fucking love it...And I assume the majority of people haven't even seen the film, let alone played the game.
The Neverhood for PC. I can't believe I missed this game as a kid. I played it recently and I'm in love. I wish more people could play it.
Games you know no one else has or even heard of...

Love both these games! VIB Ribbon had cool songs in it. It would also create a level based on your music CD. Simple button mash react to screen line image, but very cool.
Here is a Freeware game VIB inspired for the PC:

Neverhood is a classic! Have both PC and PS1 Kayaman Klayman versions.

Okay here's another to take you waaaaay back.
Bilestoad for the Apple II+
http://en.wikipedia.org/wiki/The_Bilestoad

Simple graphics, each arm is controlled independently using keyboard only, whew!
The game characters were called meatlings and you can hack off an arm, opponent would still be alive.
When you get hurt that bad and can't fight, run to distant warp pad dripping blood all the way ................

Hmm, let's see, some games folks might have missed...
Rakuga Kids: It's a fighting game where you play as doodles drawn by kids with magic crayons. It's unfortunate this game never made it to the states, but it did see a release in Europe.

There's also S.D. Hiryu no Ken Densetsu, also for the Nintendo 64. If you ever played Flying Dragon, then you've got the idea. It took the super deformed part of Flying Dragon and turned it into a bigger game, with more items and RPG elements.

I think Nathan should get Trale Lewous and Keith Apicary to do some combined episode about this game.
It was out on PC as well which is where I played it. The dialog was fun and light and most of the gameplay was ok, but it was hard to gauge where your jump was going to land and I gave up on it in a level where I landed in the water and died too many times.
Granted I read about it on some site and picked it up myself, but Operation Logic Bomb is a pretty cool SNES game.

I would say most of my game collection. Some people know of some of the more well known games but some of them, especially the older games, no one I talk to has even heard of them. Then again I don't know many people that still play old games...
NES, SNES, PS1, N64, DC, PS2, GCN, Wii, PS3, 360, GB, GBC, GBA, GBA SP, PSP 3000, and 3DS XL

I throw another one up there for Stretch Panic. I bought it because it sounded so bizarre and I didn't quite understand it. It's not so much a game as it is an incomplete toy. I think it's fun to play around with in the same way noby noby boy is, the amusement just doesn't last as long. I also recently picked up a japanese game called 1 on 1 which is a half court 1 on 1 basketball game. It plays sort of like a fighting game. The characters are very bizarre. One of them has a tail and looks like a monkey, and a skinny pale gothic guy with wings. I haven't figured out exactly how to play yet, but so far it looks like you just beat the crap out of eachother till you get the ball, and then try to score. Anyone else know about this one?

Still need to get Rakugaki Showtime. It's up on the Japanese PSN as a PSone Classic.
