There may have been some that didn't have an "erase" option in the menu, but I don't think there were any that couldn't be erased using a backup unit. Most games without this option would have used the same type of save RAM as any other N64 cart.

If I understand correctly, Capcom is claiming that they didn't merely leave out the save option in the program, but rather the save area on the cartridge itself isn't physically capable of being written to more than once per sector (or something like that). We'll see how true this is once someone releases a save manager for 3DS.

You had to hold down start as the system was turned on to access the save management screen of some games. I'm sure it said so in the instruction booklet. It was probably just to prevent little kids from accidentally deleting saves.

You're correct. Games like Blast Corps used this method.

This was only for games that used the N64's controller pak. It was the only way to manage saves on the device.

Actually some games had a regular menu that let you alter files on the controller pack. Diddy Kong Racing is one such game.
